Key Responsibilities:
Provide confidential, non-medical counseling services to patients dealing with chronic pain, helping them navigate and process their experiences.
Create and implement individualized treatment plans tailored to meet the unique needs of each client.
Assess participants’ immediate risk factors and evaluate their biopsychosocial needs to provide effective support.
Deliver educational resources and support to patients and their caregivers, enhancing understanding and management of their conditions.
Serve as the primary liaison for patients and healthcare professionals, ensuring seamless communication and coordinated care.
Understand each patient’s support needs and interaction preferences, providing a personalized experience that helps them adhere to prescribed treatment plans.
Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and assessments, ensuring compliance with all clinical standards.
Requirements
Qualifications
Master's degree in Social Work from an accredited institution
Got a valid and active CA state license to practice as a Licensed Clinical Social Worker (LCSW)
Minimum of 2 years of clinical experience to the table, with a focus on providing psychotherapy services
