Overview:

DePaul is now hiring a full-time Intake Specialist in Depew, NY! This position works under the supervision of the Intake Director, to process referrals for potential placement for all licensed housing programs. This includes all necessary steps from pre-placement assessment to admission into the program.

Responsibilities:

Assist the Director with generating agency referrals by making public contacts such as presentations, mailings, and community involvement.

Conducts and / or coordinates designated referrals for pre-intake screenings, tours (as needed), and dinner visits.

Makes certain to arrange and / or provide transportation to and from site visits during the screening and / or move in process.

Reviews and gathers all necessary / required referral material and documents this information into the database.

Serve as an agency representative on various Single Point of Entry County meetings as instructed by the Director.

Attend and participate in weekly Risk Management meetings when necessary to review and discuss potential referrals as well as to remain aware of the individuals’ housing plan.

Stays knowledgeable of all agency programs, their admission criteria, and any applicable changes that may occur.

Utilize person centered practices and information gathered throughout the assessment process to make recommendations for placement.

Respects and maintains resident confidentiality and demonstrates a caring and positive attitude toward all residents, family members, staff, and service providers.

Works closely with the Director to monitor the status of referrals, the waiting list, and any potential resident movement within the agency.

Serves as a representative on various committees both within the agency and outside the agency in order to promote teamwork and community involvement.

Works with the Director to ensure that a disposition in writing is submitted for all referral applications within the required time frame. 

Performs any other duties necessary for the effective and efficient operation of the Intake Department.

Qualifications:

Bachelor’s degree in Psychology or Human Services field required, with one year experience working with individuals with psychiatric disabilities (preferably in a residential setting) preferred or an Associate’s degree in Human Services field with two-years of experience working with individuals with psychiatric disabilities (preferably in a residential setting).

Other:  In addition to the above criteria, this position also requires a clean NYS drivers license as outlined in DePaul’s clean driving record policy.  The use of ones personal vehicle is also a requirement.
