Advanced Practice Nurse, Palliative Care
VNA Health Group is is moving healthcare forward. We are looking for an Advanced Practice Nurse, Palliative Care to join our team in Essex County. The APN, Palliative Care will be responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for recently diagnosed patients with advanced or chronic serious illness.

Responsibilities:
Principal Responsibilities:
Supports organization’s mission by striving for excellence in all aspects of his/her job with a focus on positive interpersonal relationship with co-workers
Assesses, diagnoses and develops appropriate plan of care to treat palliative care patients in a coordinated fashion
Serves as a liaison for care coordination across VNA continuum for patients with chronic complex illness
Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations
Adheres to agency infection control and safety policies
Adheres to the organization’s policy in regards to absenteeism and appearance
Able to provide care in hospital, facilities and homes.
Qualifications:
Job Competencies & Minimum Qualifications:
Master’s degree in Nursing required from an NLN accredited program
Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA
National certification as an Adult, Geriatric or Hospice and Palliative Care Nurse Practitioner
1-2 years of APN experience preferred
Excellent communication, organizational and interpersonal skills
Strong computer skills
Working Conditions/Physical Demand: Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver’s license and automobile insurance
