Program Description

Felton Outpatient Services Solano (FOSS) is a multi-disciplinary team of clinicians, medical providers, case managers, and peer advisors that provide recovery-oriented, culturally responsive, and trauma-informed manner specialty behavioral health services to clients 18 and older (moderate level intensity), who are Solano residents and/or have Solano County Medi-Cal or Kaiser Medi-Cal.  The team will assist individuals experiencing severe mental illnesses that interfere with their ability to live in the community, attend appointments with professionals in clinics and hospitals, and manage mental health symptoms by providing the following outpatient services: mental health services (i.e., assessment, evaluation, plan development, collateral, family engagement, individual and group therapy, individual and group rehabilitation, and interactive complexity), case management/brokerage, crisis intervention, and medication support.

Job Description

The Clinical Case Manager is a licensed or is a BBS registered clinician with direct service experiences with individuals experiencing homelessness, substance use, mental health needs, and medical complexity. Referrals for the program caseload come from Solano County BHCS and the caseload ranges up to 35-40 clients to provide the intensive services needed. The primary functions include both direct service and interdisciplinary/multi-agency collaboration. Local travel between agency sites as well as direct service provision to program participants is expected.

Specific Responsibilities

Provides trauma informed, culturally responsive services that include mental health services including modalities ranging harm reduction, contingency management, motivational interviewing, brief solution focused therapy, crisis intervention.

Elicits and maintains strong working relationships with clients and the care team which may include other professionals, peers, medical providers, and client identified support persons such as chosen family, friends, or others

Facilitates and tracks linkages, care coordination, and case management among medical and community- based supports

Outreaches to shelters, streets, hospitals, and community settings to meet clients where they are at and able to locate clients by use of electronic health record systems and planning with client

Ensures timely and accurate documentation of all services provided and maintains applicable chart requirements

Provides inter-agency collaboration to facilitate problem solving and clinical care

On-call consult during working hours to triage clinical emergencies and coordination for coverage to the team

Serve as a back-up for the team as needed

Attend and participate in all assigned trainings for professional, program, and agency development.

Additional duties as needed.

Minimum Qualification

Master’s Degree (or higher) in Social Sciences, Psychology, Counseling Psychology or related field AND current California State Licensure or registered as an associate. One year post-graduate experience in similar setting preferred

Two years of experience providing mental health services to individuals with serious mental health challenges, substance use, and homelessness

Experience with specific modalities harm reduction, contingency management, substance use disorder treatment, crisis intervention, de-escalation, and motivational interviewing

Outreach in settings that include shelters, street encampments, SROs, hospitals, and other areas as assigned

Charting and documentation experience under insurance payors such as medi-care, medical, and special population funding

Demonstrated proficiency in Microsoft applications, phone based applications, and navigating EHR systems required (EPIC and CIRCE experience preferred)

Competencies

Strong working knowledge of mental health and substance use disorders with DSM diagnostic skills (clinical interviewing, assessment, structured diagnostic tools where appropriate, and use of available medical records to inform assessment)

Ability to provide psycho-education to clients and consultation to community partners to support the client in their recovery process

Demonstrated ability to establish trusting, hopeful relationships with clients and community supports with respect of differences of opinion, beliefs, culture, appearances and ways of life.

Excellent communication (written and verbal) and interpersonal skills.

Adaptable, open to change, and able to tolerate ambiguity.

Strong organizational skills, attention to detail, ability to prioritize tasks, and manage time effectively.

Demonstrated experience as a team player with a welcoming attitude.

Work Schedule: Onsite

FLSA Status: Exempt

Salary Range: $90,000-$105,000 Annually (DOE)
