Overview:

The Community Health Worker (CHW) is responsible for providing basic lay navigation including outreach and education to community members and health screening sites.  The CHW identifies and reduces barriers to care; provides referrals to necessary resources; delivers patient reminders and follow up; recruits age-eligible uninsured or underinsured individuals without regular medical checkups for cancer screenings.  Fulfills grant requirements as specified by funders including, but not limited to: the New Jersey Cancer Education & Early Detection Program and the Comprehensive Cancer Control Program.

Responsibilities:

Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.

Identifies and reduces barriers to care; provides referrals to necessary resources; recruits age-eligible individuals for cancer screenings and CFHI program services.

Supports and refers community members to a place they call their “medical home”; connect persons who lack a “medical home” to such a facility; conducts outreach activities on cancer and chronic disease programs within targeted areas.

Prepares/Conducts in person (virtual where appropriate) outreach, educational and screening activities on HPV Immunization, lung, colorectal, prostate, cervical, breast cancer and survivorship in targeted areas.

Tracks client outcomes and provides information to authorized personnel as requested, using designated electronic devices and software tools. Assist with scheduling patient appointments as necessary.

Work with participants to address social determinants of health in order to support the chronic disease programs in an effort to reduce disparities in cancer screenings and access to care.

Adheres to the VNAHG policies, including those for infection control, absenteeism, appearance, and trauma informed resilience oriented care and safety

Qualifications:

Job Competencies & Minimum Qualifications:

High School diploma/GED equivalency required

Minimum one year of community involvement and/or working with clients, or an equivalent combination of education and experience

Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population

Bilingual; Spanish preferred

Excellent verbal and written communication skills

Ability to work both independently and as part of a team; Strong organizational and interpersonal skills

Valid N.J. driver’s license and reliable transportation

Working Conditions/Physical Demand: Various community settings both indoor and outdoor, use a laptop, cell phone, other office machines such as fax, copy, and scanner
