Overview:
Monmouth ACTS Community Health Worker will assist with in person client intakes, connecting clients to community resources, community outreach to residents, connecting with new community resources and service providers.
Responsibilities:
Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers, patients and members of the community
Maintains expertise in eligibility, enrollment and program specifications for community resource
Works with individuals and organizations that provide client services or resources (i.e. transportation services, counselors, language translators, and representatives who assist with financial support);
Adheres to agency infection control and safety policies
Adheres to the organization’s policy in regards to absenteeism and appearance.
Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
Qualifications:
Education: High school diploma or equivalent
2+years of work experience, ideally involving community. Individuals and organizations
Strong computer skills including mobile device, Apps and databases
Car, driver’s license and insurance required
Excellent verbal and written communication and interpersonal skills
Strong organizational and data collection skills
Bilingual preferred
