Responsibilities:

The Manager, Social Work, Care Management, will focus on Norton Healthcare acute care settings, in a manner that leverages training, relationships, and innovation to improve the overall wellbeing of our patients, families, organization, and community. The manager will adhere to and promote the NASW code of ethics and NASW standards for practice in a healthcare setting. Support and coordinate social worker activities across the Norton Healthcare system. Advocate for patients, families, providers, and social workers. Inspire social workers to provide excellent, creative, and innovative care. Empower social workers with mechanisms, structures and resources that support high quality care. Assist social workers with individual professional development, and elevate the practice of social workers across the Norton Healthcare system. The manager also fosters social work partnerships across Norton Healthcare and with allied community partners, while promoting quality by measuring analytics and evidence informed initiatives. In addition, the manager will lead and provide direct support with projects requiring social work consultation and service interventions.

Key Accountabilities:

Collaborate with internal NHC departments and community partners to bridge gap between inpatient and outpatient care. Collaborate with NHC, the community, and payors to provide solutions and interventions to help drive efficient and effective patient outcomes.

Oversee the overall work structure of the inpatient social work teams. This includes, but is not limited to, staff mentorship during orientation and ongoing coaching/education to ensure the effective coordination by the social work role. Manager will educate on National Association of Social Workers (NASW) standards to support evidenced-based practice is executed by all inpatient social work team members through standardized, inpatient work flows and cross-training experiences. Collaborate with community partners, hospital care management, ambulatory nurse navigators, managed care/payors and other key stakeholders to maintain and improve quality of care and patient satisfaction.

Utilize quantitative and qualitative data to drive social work initiates aimed at: Addressing social determinants across the care continuum, implementing evidence-based practice to meet patient needs and system goals (benchmarking against national standards) and leveraging social work skills sets to optimize care outcomes.

Develop and provide continuing education programs approved by the Kentucky Board of Social Work to support Norton healthcare's social work workforce. Maintain ongoing Kentucky Board of Social Work program approval. Maintain practicum program with local university students who desire clinical rotations at Norton Healthcare. Act as a supervisor for incoming certified social workers who require supervision hours and facilitate professional development/mentorship pathways.

Qualifications:

Required:

At least three years clinical experience

Master Degree

Licensed Clinical Social Worker

Previous leadership experience
