Overview:

The Social Work Care Manager is responsible for the coordination of care and case management services for patients with complex psychosocial needs while utilizing appropriate resources, both internal and external to the organization, to achieve high-quality and cost-effective solutions. The Care Manager provides social services support by partnering with clinic staff, external partners, and patients to assess and address social determinants of health and barriers that impact the patient’s wellbeing with the goal of improving positive health outcomes.

This is a clinic-based position located in the Midtown area of Atlanta, GA. (2140 Peachtree Rd.) General work hours are Mon - Fri, 7:30AM-5:30PM. Schedule will be 4/10-hour work days.

About the Role:

Perform social assessments, which include barriers to coping, cognitive development, residential stability, health system impediments, social support and vulnerability

Provide appropriate interventions which demonstrate knowledge of and sensitivity toward cultural diversity and religious, developmental, health literacy, and educational backgrounds of the population served

Assist and participate in multiple facets of meeting patient needs, including eligibility screening for various community, federal, state programs and resources available to patients. Assist patients with processing paperwork and collaborate with Economic Support Services as needed

Collaborate with clinical staff and patient/family to develop and execute patient/family centered care plans and goals integrating referrals to appropriate community resources into care plan

Assist patient, family, social relationships, medical staff with care coordination, problem solving with goal to improve the patient’s utilization of healthcare.

Attend patient appointments as needed

Establish collaborative relationships with AvitaCare Atlanta clinic staff and external professionals

Participate in community outreach, providing educational opportunities to the public and propose community outreach contacts that can enhance patient care

Participate in appropriate ongoing education and professional development

Other duties as assigned

About You:

Bachelor’s degree or higher in public health, social work, human services or related health care field highly desired

Or equivalent experience of 3 years’ experience in serving the needs of complex populations (e.g., Medicaid, Medicare, uninsured), preferably in a healthcare or social services setting

Clinically licensed as an LMSW in the state of Georgia with 3+ years of experience in serving the needs of complex populatons preferred

Knowledge of Ryan White HIV/AIDS program is preferred

Extensive knowledge of local community resources and entitlement/application processes

Proficiency with EMR, eClinicalWorks preferred

Strong written, organization, communication, and problem-solving skills required

Ability to work in a team environment
