Responsibilities:
Serves as the Medical Records Consultant and a knowledge resource to each of the FutureCare facilities and Medical Records Coordinators
Perform quarterly facility visits
Frequent maintenance of active and discharge medical records for ease of retrieval and integrity of the resident’s medical record
Create a system for retrieval of discharge medical records in accordance with policies and procedures including the Discharge Chart Control Log, and the log of records stored off-site (Medical Records Discharge File Log)
Safeguard against loss, destruction, or un-authorized use; maintained information system security and privacy
Establish a systematic process for reviewing documentation on an ongoing basis for quality and quantity of documentation
Creates and manage a request letter management database for tracking the process and the specific due dates and deadlines of various regulatory, legal and reimbursement audits and record requests from Medicare, Medicaid and other payer sources
Identify and compile documents needed to fulfill the audit / record request
Conducts quality assurance audits on a monthly basis to assure compliance with policies and procedures. Develops and evaluates performance improvement plans as indicated.
Assists in training and orientation to medical records and unit clerk staff on functions of the department
Obtains necessary information on residents as needed and other requests for residents and/or health care decision makers
Participates in data collection for the facility, including retrieval of clinical records as requested by Medicaid reviewers, State surveyors, and administrative requests for other studies or reviews
Assures timely and accurate response to all requests for medical records in accordance with state and federal laws
Abides by the standards identified in FutureCare’s Statement of Corporate Ethics and the Corporate Compliance Plan
Qualifications:
Maintain AHIMA certification and credentialing as a Registered Health Information Administrator (RHIA) or a Registered Health Information Technician (RHIT)
Minimum five years of experience in long term care
Understanding of payment systems and eligibility requirements for all payer sources, including Medicare and Medicaid
Knowledge of documentation and legal issues pertaining to health information
Knowledge of regulations, survey process, accreditation standards and professional standards of practice pertaining to health information in long term care
Able to respond timely and accurately to a multitude of information requests.
Knowledge of quality assurance and the ability to apply a quality improvement process to problem
Accuracy with details, dependability and organization
Ability to teach using a variety of methods, instruct and provide feedback
Computer skills and understanding of electronic information systems used in long term care
Ability to assist as facilities move toward an electronic medical record
