What you’ll do

Support the clinical home health and/or hospice teams to utilize a collaborative process to review, plan, monitor, and evaluate the options and services required to meet patients’ needs while coordinating available resources to promote quality and cost-effective outcomes. Specifically, we will look to you to collaborate to manage processes, protocols, and timeliness of all referrals to meet the comprehensive needs of the patients.

What it takes

High school diploma or equivalent required

Current Ohio LPN license required

Associate degree in health information or related field preferred

Two years of medical records experience in a long-term care, hospital, or other related healthcare setting required
