JOB SUMMARY
Community Health Workers play a vital role in San Antonio Regional Hospital’s Community Health Improvement Program by providing outreach, education, and support to individuals and families in the community, particularly those with chronic and complex illnesses or experiencing homelessness. The CHW for Enhanced Care Management is responsible for providing comprehensive support to patients with complex health needs. This role includes conducting home visits, offering home care, and enrolling eligible patients into the Enhanced Care Management through the Inland Empire Health Plan (IEHP) and/or Molina Healthcare program. The CHW will work directly at patients’ locations, ensuring they receive the necessary care and resources to manage their health effectively.
MINIMUM QUALIFICATIONS
Education: High school diploma or equivalent required. Associate’s or Bachelor’s degree in a related field preferred.
Experience: Experience working with diverse populations, particularly those with complex health and social needs.
Knowledge and Skills: Knowledge of local healthcare and social service systems. Strong interpersonal and communication skills, with the ability to build rapport and trust with patients. Ability to work independently and as part of a multidisciplinary team. Excellent organizational and time-management skills. Cultural competence and sensitivity to the needs of diverse populations. Proficiency in Microsoft Office and Electronic Health Record (EHR).
License/Certifications: Certification as a Community Health Worker (CHW) required. Valid CA driver’s license and reliable transportation.
Equipment: Proficient in Microsoft Office applications.
Physical Requirements: Must be able to perform the essential physical requirements of the job. Exposure to diverse environmental conditions. Combination of office and field work, including home visits and community settings.
PAY RANGE
$22.00 - $30.57
The posted pay range reflects the lowest to highest pay that was available for this position at the time of posting and may be subject to change. Salary offers are determined by candidate’s relevant experience and skills. For per diem positions, a standard rate is used based on market data and not the candidate’s individual experience.
