This position involves assessing patients' health conditions, administering medications, and collaborating with interdisciplinary teams to ensure comprehensive care. Your expertise will be instrumental in promoting the health and well-being of patients recovering from surgery, managing chronic illnesses, or requiring assistance with daily activities. Responsibilities

Conduct thorough assessments of patients' health status in their home environment.

Develop and implement individualized care plans based on patient needs and goals.

Administer medications and treatments as prescribed by physicians.

Monitor patients' progress and maintain accurate medical records.

Educate patients and their families about health management and care practices.

Coordinate with interdisciplinary teams to provide holistic patient care.

Provide emotional support and guidance to patients and their families.

Requirements

Current and valid RN license in the state of practice.

HomeCare Homebase Charting Experience Required

Minimum of 1 years of direct home health experience, within the last 3 years.

Strong clinical assessment and decision-making skills.

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Basic life support (BLS) certification required.
