Monday - Friday, No Weekends, No Call required.
Key Responsibilities
Perform comprehensive assessments of patients in their home environments.
Design, implement, and modify individualized care plans in collaboration with the healthcare team.
Administer prescribed medications and treatments appropriately and safely.
Monitor and document patients' progress, making adjustments as necessary.
Provide education to patients and families on health conditions and care plans.
Facilitate communication and coordination among team members and other healthcare providers.
Offer emotional support and guidance to patients and families throughout the care process.
Requirements
Current RN license in the state of practice.
Experience with HomeCare Homebase Charting required.
Strong clinical assessment skills and critical thinking capabilities.
Excellent communication skills and the ability to build rapport with patients.
Self-motivated with strong time management skills.
BLS certification required.
OASIS experience required.
Must have at least 12 months of home health experience within the past 3 years.
Must meet minimum productivity expectation of 30 points per week.
