IMC Locums are currently recruiting for a Community Physiotherapist to join our team on a locum basis. As a Community Physiotherapist, you will be responsible for providing high-quality care to patients in their homes, care facilities, or community clinics. You will work autonomously and as part of a multidisciplinary team (MDT) to support individuals with mobility, rehabilitation, and independence, often following hospital discharge or in managing long-term conditions.

Key Responsibilities:
• To provide high quality patient-centred care and specialist Assessment and treatment to persons referred to the West Mendip Community Rehabilitation service (CRS).
• To provide physiotherapy treatment and advice for patients referred to the team
• Community rehab experience, to include falls, frailty, MSK, Neuro.
• Participate in Triage where patients are triaged and assessed as appropriate for the CRS team or referred onto more appropriate services
• To provide advice regarding the support and treatment of patients, to families, carers and other members of the MDT.
• Responsibility for a caseload of patients and refer on or discharge as necessary
• Carry out triage autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the team
• To support junior staff and students as appropriate

Requirements:
• Degree in Physiotherapy (BSc or equivalent).
• Full registration with the Health and Care Professions Council (HCPC).
• Previous experience working in a community physiotherapy setting.
• Ability to work independently, manage your own caseload, and make clinical decisions.
• Excellent communication and interpersonal skills, with a patient-centred approach.
• Full UK driving license and access to a car

Desirable:
• Previous experience in a locum or agency role.
• Experience working with elderly patients or those with chronic conditions.
• Knowledge of NHS systems and pathways for community rehabilitation.
