Responsibilities:
The Licensed Practical Nurse provides intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. As a Licensed Practical Nurse you will:
Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care
Submit completed skilled nursing visit notes and participates in the discharge planning process per Medicare Criteria
Provide assistance with activities of daily living (ADL’s) which may include personal hygiene, changing of bed linens, dressing and ambulation and transfers as the patient need arises
Attend staff meetings, team conferences and educational in-services, as needed
Participate in Process Improvement (PI) program by assisting with collection of data
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications:
Qualifications
A current unencumbered State Professional Nurse License
Active CPR Certification
Must maintain a valid driver's license, reliable automobile, and good driving record
Proficiency in clinical skills
Proficiency in Microsoft Office
