Acts Hospice is currently seeking qualified candidates for our Social Worker position.

In this role, you will be responsible for developing, providing, documenting, and evaluating individualized social work assessment, counseling, and casework services to hospice participants and families as directed by the plan of care. Counsels hospice participants and families on issues related to self-determined life closure, safe and comfortable dying, and effective grieving, in collaboration with the Interdisciplinary Team (IDT).

Requirements:

The ideal candidate will meet the following requirements:

Graduate degree in social work or related field

Current licensure in the state where services are rendered

Minimum of two years' experience in hospice or a transferable, clinical venue of social work practice such as oncology, geriatrics, or community health
