Program Manager - Care Navigation and Disease Management

1.0FTE, 40 hours per week, Salary, Exempt

Plans, implements, and evaluates the administrative, operational, supervisory and direct care activities for an assigned cost center. Leads a multi-disciplinary team of professional and support staff in strategic directives and care activities to ensure the assigned department achieves best outcomes for patients, families and staff.

Responsibilities:

Recruits, trains, evaluate, develops, disciplines and/or terminates subordinate staff. Assures that performance standards for all staff are met.  

Promotes teamwork by building a team environment and fostering cross functional collaboration.  

Ensures regulatory compliance and emergency response preparedness.    

Maintains safety and quality as priorities, providing direction, prioritization, accountability, enthusiasm and support for safety and quality initiatives.  

Coordinates staff representation on departmental, hospital and community task forces, committees, quality teams and advisory groups. Serves as a representative to such groups when appropriate.  

Coordinates the design of educational material and the orientation of department personnel to case management role functions as well as information systems.  Assists and directs staff in the identification of their own learning needs and assists staff in meeting those needs.   

Oversees the clinical case management within the department.  Maintains a working knowledge of assigned functions.  Serves as a liaison with third party payors, community agencies and leadership regarding these essential functions. 

Assumes responsibility for the operation of the department in the Director’s absence as needed. 

Contributes to development of expense, operational and capital budgets.  

Note: Regular, reliable and consistent attendance is an essential job function. Employees are expected to perform work as scheduled. Please see the physical requirements for this position listed below.

Qualifications:

Education:

MSN or Master's Degree in related field required

BSN required

Licensure:

Licensed as a Registered Nurse in the State of Ohio required

Valid Ohio Driver's License required

Case Management certification preferred

Experience:

Three years pediatric clinical experience or equivalent combination of education/experience required.

Driving is required using your own vechicle.
