Our client Herefordshire Council is looking for an Adult social worker to join their Transitions team.
The Young Adults (Transitions) Team supports young people aged 18-25 as they move from children into adult social care. Working in line with the Care Act 2014, you will complete strengths‑based assessments, identify eligible needs, and develop care and support plans that promote wellbeing and independence. The role also involves Section 42 safeguarding enquiries, CHC, Mental Capacity Act assessments and best‑interest decision‑making, and contributing to court work as required, working closely with young adults, families, and multi‑agency partners to ensure safe and positive transitions into adult services.Benefits of working with Remedy
