A specialist complex care team is seeking a Children’s Nurse Case Manager to join their dedicated workforce. In this role, the successful candidate will take clinical ownership of a caseload of children with complex needs.

Key Responsibilities:

* Lead comprehensive assessments and oversee care delivery for children with complex health conditions.

* Support and guide frontline staff in delivering high-quality care.

* Serve as the primary clinical point of contact for families, commissioners, and healthcare professionals.
