Your responsibilities will include completing moving and handling assessments, Disabled Facilities Grant (DFG) assessments, Blue Badge assessments, and recommending appropriate equipment and adaptations. You'll work closely with colleagues and partner agencies to deliver person-centred support across the community.

Flexibility around working arrangements is available, however regular community visits and attendance at team meetings are an essential part of the role.
