Social Worker – Early Help & Short Breaks Team | Liverpool City Council | Up to £35 per hour
Sheldon Phillips are working with Liverpool City Council to recruit an experienced Social Worker to join their Early Help and Short Breaks Team. This is a rewarding opportunity to support children with disabilities and their families through targeted interventions, assessments, and support planning.
You will work with children with disabilities and their families to assess needs, coordinate support services, and provide access to short breaks and specialist resources. The role focuses on promoting positive outcomes, family resilience, and preventing escalation to statutory intervention where appropriate.
Key Responsibilities
Undertake Child and Family Assessments for children with disabilities.
Complete Short Breaks Assessments and Carers Assessments.
Develop and review support plans tailored to individual family needs.
Work closely with education, health, and community partners.
Support families to access appropriate services and resources.
Maintain accurate case recordings and assessments.
Promote safeguarding and ensure children's welfare remains central to all interventions.
