We are currently recruiting for an experienced Adults Social Worker to join Norfolk County Council's dedicated Hospital Discharge Team based at Norfolk and Norwich University Hospital (NNUH).
This is an excellent opportunity to work within a fast-paced, multi-disciplinary environment, supporting adults to achieve positive outcomes through effective discharge planning and person-centred social work practice. Working within a Home First model, you will play a key role in promoting independence, reducing unnecessary hospital stays, and ensuring individuals receive the right support at the right time.
Key ResponsibilitiesAs an Adults Social Worker, you will:

Undertake strengths-based and person-centred assessments with patients and their families.
Develop, implement, and review care and support plans that maximise independence and wellbeing.
Work closely with hospital staff, health professionals, and community services to facilitate safe and timely hospital discharges.
Apply legislation including the Care Act 2014, Mental Capacity Act, and associated statutory guidance.
Complete Mental Capacity Assessments and Best Interest decision-making processes where required.
Support Continuing Healthcare (CHC) assessments and reviews.
Coordinate services and interventions to ensure smooth transitions from hospital to community settings.
Maintain accurate and timely case recordings and assessments.
Participate in post-discharge reviews for individuals returning home or moving into care settings.
Contribute to a collaborative and supportive team environment.RequirementsTo be considered for this role, you must have:

Social Work England registration.
A recognised Social Work qualification (Degree/DipSW/CQSW).
Previous experience within Adult Social Care.
Experience working within hospital discharge teams or similar settings.
Sound knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures.
Excellent assessment, communication, and partnership-working skills.
A full UK driving licence and access to a vehicle (travel required).What's on Offer?
