Job Description – Support Worker (Supported Living)

* Supporting adults with learning disabilities, autism, mental health conditions, dementia, epilepsy, and complex needs within supported living settings

* Assisting with personal care, medication administration, meal preparation, and daily living activities

* Promoting independence, dignity, choice, and person-centred support

* Supporting service users to access community activities, appointments, and social engagement

* Maintaining accurate daily notes, incident reports, and care documentation

* Following safeguarding procedures and reporting concerns appropriately

* Supporting individuals with behaviours that challenge using positive behaviour support approaches

* Working in accordance with health & safety, infection control, and company policies

* Working collaboratively with healthcare professionals, families, and multidisciplinary teams

* Providing emotional support and encouraging wellbeing in a safe and supportive environment

Requirements:

* Minimum 6 months experience in Supported Living / Care settings

* Valid Right to Work in the UK

* Care Certificate including all 16 modules

* PMVA / MAPA / CPI Training Certificate

* Enhanced DBS registered on the Update Service

* Good communication and teamwork skills

* Ability to work flexible shifts including days, nights, and weekends

* Understanding of safeguarding and person-centred care principles
