About the Role
We are currently seeking a dedicated and experienced Team Leader / Rehabilitation Support Worker to support an 18-year-old male client living in the CR3 area.
This is a highly rewarding opportunity to work within a dynamic multidisciplinary team (MDT), supporting a young individual with complex needs through a structured rehabilitation pathway.
The role focuses on neurorehabilitation and spinal injury support, bridging the transition from hospital to home. You will play a key role in delivering tailored care plans, promoting recovery, and enhancing the client’s independence and quality of life.
Key Responsibilities
* Deliver high-quality, person-centred care in the client’s home
* Support rehabilitation programmes and therapy goals
* Assist with manual handling and mobility support
* Administer and monitor medication safely
* Work collaboratively with healthcare professionals and therapists
* Monitor progress and report changes in condition
* Provide emotional and psychological support to the client
* Help prevent complications and promote long-term wellbeing
Person Specification
Essential Skills & Experience
* Experience supporting individuals with spinal cord injuries and complex needs
* Strong manual handling skills
* Experience in medication administration and monitoring (hospital/ICU background desirable)
* Understanding of rehabilitation and recovery pathways
* Excellent communication and interpersonal skills
* Good IT and documentation skills
* Ability to work independently within a client’s home
* Willingness to learn, adapt, and take feedback
Desirable
* Experience supporting clients in community-based settings
