By the order of the Physician or Case Manager, assesses the discharge, social service needs of the elderly, disabled, financial needy patients, including counseling those with risk for financial/emotional/physical abuse/ exploitation. Makes referrals to approach social service agencies and arrange for placement as directed. Consults with MSW SW as needed for clinically complex patients or difficult family dynamics.

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

BSW Reqiuired (Social Work, Professional Counseling, Marriage, Family and Child Counseling)

1-3 years of experience in Social Work with emphasis on discharge planning, referral to community resources and/or case management.

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10

lbs. constantly

This BSW Social Worker role works from 8:00am to 4:30pm at St Anthony's.  We are looking for candidates with a minimum of 2 years of Social Work experience.

With four trauma rooms including the T-10 room, a dedicated field-to-surgery suite ready 24/7 for trauma surgeons and specially trained teams, our staff can provide life-saving care to the most severely ill and injured patients. In 2023, we received our exciting Magnet® designation!
