Seasoned Recruitment is seeking a qualified and compassionate Primary Care Nurse Practitioner (FNP or AGNP) to join our dynamic healthcare team. As a Nurse Practitioner, you will provide comprehensive, patient-centered care to a diverse population. Your role will involve conducting assessments, diagnosing illnesses, providing treatment plans, and collaborating with a multidisciplinary team to ensure quality care for our patients.

Salary/Wage base range for this role is $145,000 - $175,000 / year + Bonus + Equity.
Key Responsibilities:
Perform thorough physical exams and patient assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participantsDiagnose and manage acute and chronic illnesses.
Work closely with other healthcare professionals to coordinate care and IDT to oversee and ensure effective care coordination.
Document all patient interactions accurately in the electronic medical record (EMR).
Participate in continuous quality improvement initiatives.
Must collaborate with physicians and IDT to develop care plans for each participant
Must collaborate with physicians and IDT in discharge planning from acute or long-term settings
Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
Collaborate closely with physicians, community-based PACE physicians, and the Interdisciplinary Team (IDT) to supervise and guarantee effective care coordination.
At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
Take part in rotating night and weekend call
Requirements
Qualifications:
Graduation from an Accredited School of Nursing
Certificate as a Nurse Practitioner with Prescriptive Authority
Active Drug Enforcement Administration (DEA)
Current knowledge of chronic care/geriatric issues and best practices
At least 2 years of clinical experience in the senior population
Must currently be enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
Valid CA. state drivers license
Exceptional communication and interpersonal skills
