The Speech Language Pathologist assesses, plans, implements, provides and supervises individual patient care according to professional standards and departmental policies and procedures. Performs other duties as assigned or requested in order to maintain a high level of service.

1. Consistently applies therapeutic principles and practices during patient evaluations and treatments.
2. Formulates realistic and appropriate plan of cares as well as short and long term goals.
3. Involves patient/family in program/ treatment decisions and plans.
4. Terminates treatment appropriately with discharge plans.
5. Shows sensitivity to cultural backgrounds
6. Maintains timely and accurate documentation and charging in accordance with department policy.
7. Regularly assesses needs for and provides patient/family with educational materials and resources including home exercise plan.
8. Actively participates in quality improvement initiatives.
9. Functions as a preceptor to new staff as well as supervises clinical students.
10. Takes responsibilities to monitor own schedule and follow up with appropriate staff as needed.
11. Complies with organizational and departmental policies and procedures
12. Maintains productivity at departmental standards.
13. Meets performance expectations for Customer Service, Teamwork, Resource Utilization as well as Staff and Self Development. Abides by the YNHH Standards of Professional Behavior
14. Performs other duties as assigned or directed to ensure smooth operation of the department.
Qualifications:
EDUCATION
M.A., M.S., Or M.Ed. In Speech-Language Pathology
EXPERIENCE
Outpatient experience preferred
LICENSURE
Connecticut Speech-Language Pathology license or Speech-Language Pathology of CFY
SPECIAL SKILLS
Certificate of Clinical Competence (CCC) qualification in Speech-Language Pathology by American Speech-Language-Hearing Association (ASHA).
