Role Overview: We are seeking an experienced home health Registered Nurse who is familiar with completing OASIS assessments. The ideal candidate should be a team player who is flexible, willing to learn and teach, capable of working independently and part of a team- supervisory experience a plus! In this RN position with We Care Home Health, you will play a crucial role in supporting our clinical staff, admitting and caring for new clients, and ensuring top-level care is provided while guaranteeing their safety, comfort, and overall well-being.
Duties:
Admit new clients to the agency and complete recertifications/discharges as necessary utilizing OASIS assessments.
Complete routine client visits.
Be part of the on call team on a rotating basis to share the Supervising RN responsibilities with other RN's, and participate in the contingency plan for 24/7 clients.
Perform other duties as discussed or assigned
Qualifications:
Graduate of an accredited nursing program with a current license as a Registered Nurse in the state of Colorado
Minimum of two (2) years nursing experience in home care, and at least one (1) year of supervisory experience
Demonstrated ability to supervise, coordinate, and direct clinical personnel
Knowledge of Medicare/Medicaid and ACHC standards preferred
BLS certified
Excellent written and verbal communication skills and strong interpersonal skills, including the ability to market and relate tactfully with clients and other members of the community
Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements
