Position Overview: We are seeking a skilled and compassionate Ambulatory Care Pharmacy Technician to join our dynamic team. The Ambulatory Care Pharmacy Technician will work at Transition of Care/Skilled Nursing Facilities (TOC/SNF) for the equivalent of 5 days a week (1FTE) to support our ambulatory care services by assisting pharmacists and Transition of TOC/SNF in medication management, patient education, and ensuring optimal medication therapy outcomes.
Responsibilities:
What You Will Do
Key Responsibilities: Clinical Support: Support pharmacists and TOC/SNF in conducting Collaborative Drug Therapy Management (CDTM) and patient assessments. Document patient interactions and medication therapy outcomes including but not limited to: -Gathering medication histories with accuracy from multiple Electronic Health Records -Identifying potential errors or necessary clarifications with acute precision -Medication uses and administration -Prescribing accuracy -Administration of CDTM per protocols Administrative Duties: Assist with insurance processing and billing as needed including but not limited to: -Alleviating pharmacist and TOC/SNF staff’s time completing various administrative tasks -Delivering safe patient care
Patient Interaction: Address questions and concerns regarding and facilitate medication access.
Compliance: Ensure compliance with all relevant regulations and standards, including those set by the Board of Pharmacy and other regulatory agencies.
Collaboration: Work closely with healthcare providers to coordinate patient care and optimize medication therapy.
Skills/Abilities:
Strong communication and interpersonal skills
Ability to work effectively in a team-oriented environment
Proficient in using pharmacy software and Electronic Health Records (EHR)
Qualifications:
MINIMUM EDUCATION:
-High school diploma or equivalent
-Completion of a pharmacy technician training program
MINIMUM EXPERIENCE:
-1 year experience as a pharmacy technician
PREFERRED EXPERIENCE:
-1 year experience as a pharmacy technician in an ambulatory care setting
-1 year Collaborative Drug Therapy Management experience
REQUIRED CERTIFICATIONS/LICENSURE:
-Completion of Medication Therapy Management (MTM) and Medication History certificate through (url removed) at the time of hiring or within 6 months of hiring date
