Work Schedule:
7 days on and 7 days off, 5:30 PM to 8:30 AM, Monday through Thursday, 5:30 PM Friday and all day Saturday and Sunday, week on and week off.
Hospice Acute Care Patient RN Training provided.
Flexible work schedules: PRN, Part-Time or Full-Time, 10-hour shifts available, 30-to-40-hour work week, benefits offered for Part-Time and Full-Time employment.

Responsibilities:
This position is an RN who works outside of scheduled business hours. This person is responsible to be the field contact for Triage nursing and responds to calls as they come in while on shift. This position is responsible for completing patient visits and any required follow-up for patient/caregiver needs as well as any scheduled visits after business hours. They may plan, organize, and direct Hospice and/ or Home Health care as needed. The nurse builds from the resources of the community to plan and direct services to meet the holistic needs of patients and families with an emphasis on palliative care for hospice patients and rehabilitative care for home health patients.
Patient Care:
Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
Assesses and evaluates patient’s status by evaluating patient and family/caregiver needs, as directed and participating in revising the plan of care in collaboration with the attending physician (or approved medical provider), and/or the hospice medical director as the patient status and needs change.
Works autonomously to initiate appropriate preventive, rehabilitative or palliative care nursing procedures. Administers medications and treatments as prescribed by the physician (or approved medical provider) in the patients plan of care.
Involves the patient and family/caregiver to address needs and meet related goals.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists in facilitating the patient’s efforts toward self-sufficiency or obtaining appropriate caregiving and optimal comfort care or independent level of functioning.
Communication:
Documents patient services and status timely and accurately within professional practice standards and reimbursement requirements.
Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician (or approved medical provider) regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’( or approved medical provider) orders as required.
Collaborates with the on-call hospice provider to change the patient’s level of care as appropriate and indicated (for hospice patients only).
Communicates with community health related persons to coordinate the care plan.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician (or approved medical provider) and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group or team.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
Attends required staff meetings and interdisciplinary group meetings or case conferences.
Communicates report of services provided to the agency clinical leadership team and/ or RN Case Manager at the end of each shift.

Qualifications/Skillsets:
Graduate of an accredited school of
One to two years of recent acute care experience in an institutional
One to two years of recent experience in hospice or home health
Currently licensed as a Registered Nurse in the State(s) of planned
Possesses and maintains CPR
Communicate effectively, both written and orally, with all levels of the
Exceptional interpersonal skills both over the phone and in
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Self-directed and able to work with minimal
Capable of prolonged or considerable walking or Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Capable of considerable reaching, stooping bending, kneeling, and/or crouching. Possesses visual acuity and hearing to perform required nursing skills.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
