Responsibilities:
The Patient Care Coordinator provides clinical support to HarmonyCares Medical Group patients primarily through phone encounters. They answer inbound inquiries from patients, triaging and communicating to providers with the appropriate urgency. They communicate clinical information to patients on behalf of the provider. The Patient Care Coordinator is a member of the patient’s care team, assisting with necessary care coordination efforts and proactively monitoring patients upon instruction.
Essential Duties and Responsibilities
Provide medical triage, consistent with scope of practice and competency, for inbound clinical inquiries (via telephone calls and patient portal messages), including inquiries related to new / emerging medical conditions or sick calls, requests for new orders, and questions about medications
Maintain a paneled caseload of lower acuity patients and will be responsible for tasks such as care plans and proactive outreach
Initiate the SBAR process and utilizes appropriate triage protocols when assessing a clinical situation and providing recommendations to patients and providers
Document all phone calls and patient interactions in the EHR promptly by utilizing standard clinical documentation guidelines as required by HarmonyCares and local, state and federal rules and regulations
Review, transcribe, communicate and processes providers' verbal orders, as requested
Assist with patients' medication refill requests, via telephone requests and fax requests, including checking for duplicate refill requests and preparing necessary refill information for provider to review
Qualifications:
Required Knowledge, Skills and Experience
Ability to react decisively and quickly in emergencies
Knowledge of medications effects, indications, and contraindications
Experience with multi-tasking
Proficiency in using electronic health records (EHR) system, call center technology, and Microsoft Office Suite
Ability to communicate effectively with patients and their family members, and facility staff
Knowledge of basic principles, practices and techniques in primary healthcare
Ability to read and interpret written information, and write clearly and informatively
Ability to speak clearly and persuasively in positive or negative situations, including listening and getting clarifications, and responding well to questions
Preferred Knowledge, Skills and Experience
Active LPN/LVN license in the state where patients are located, Foreign Nursing Grad, or Foreign Medical Grad
2 years of experience as an LPN or LVN
Familiarity with Medicare and Medicaid insurances and programs
Experience in Aprima (EHR)
