Schedule: Monday - Friday, 40 hours per week.
Responsibilities
Conduct comprehensive psychosocial assessments for patients and families.
Develop and implement individualized care plans in collaboration with the healthcare team.
Provide counseling and emotional support to patients and families facing end-of-life issues.
Connect patients and families with community resources and support services as needed.
Educate families about the grieving process and provide anticipatory guidance.
Document all interactions and care provided in accordance with agency policies and regulatory requirements.
Requirements
Master's degree in Social Work from an accredited program preferred, Bachelor's degree required.
Current and valid state licensure as a social worker.
Experience in hospice, palliative care, or related fields preferred.
Strong assessment and interpersonal skills.
Ability to work collaboratively within an interdisciplinary team.
Excellent communication and counseling abilities.
Compassionate and empathetic approach to care.
Knowledge of community resources and support services.

Patient Related Travel Mileage Reimbursement
