Overview:

Performs Memorial Physician Services Pre-Visit Planning process for patient populations within the primary care setting. The Pre-Visit Planning Associate will utilize tools designed to identify care gaps before the patient attends their Primary Care visit. Primarily, the Pre-Visit Planning Associate will work in an office setting, partnering with the Primary Care Physician team to provide physicians with a check list of possible evidence based care activities to improve patient outcomes, increase provider efficiency and effectiveness, and the overall quality of care delivered.

Qualifications:

Education:

High school graduate or equivalent required; Associates degree preferred.

Experience:

One year experience in a medical office or clinical setting required. Previous clerical and EHR documentation experience preferred.

Responsibilities:

Other Knowledge/Skills/Abilities:

Strong general computer skills in Microsoft Office; especially Microsoft Excel spreadsheets, databases, and reporting tools strongly preferred.

Possesses strong medical terminology.

Demonstrates understanding of evidence based healthcare.

Demonstrates understanding of the trend toward value-based care taking place in healthcare.

Demonstrates ability to work in a fast changing and ambiguous environment.

Engaging service oriented skills required.

Excellent organizational skills required. Demonstrates ability to be flexible and function in stressful situations.

Excellent oral, written communications and interpersonal skills required.

Demonstrates initiative, self-direction, and motivation.

Demonstrates open/global communication skills with multiple requesters

Ability to work with multiple care providers and maintain positive working relationships.

Review patient care-gaps identified on the patient chart from the electronic medical record prior to pre-scheduled or same-day visit at MPS clinics. Provides patient care gaps to providers to resolve with the patient.

Review patient charts and investigate any red flags that may need further review by a provider.

Assists with MPS overall financial performance by identifying appropriate preventive healthcare tests and procedures that patients need performed.

Accountable for improving provider efficiency and effectiveness by performing pre-visit planning form; allowing the physician to spend more time with the patient.

Performs opening and close of day activities. This may include running reports and preparing for next day’s pre-scheduled patient visits.

Review patient record and abstract necessary information to complete pre-visit plan.

Functions under the direction of the Manager, Ambulatory Care Management and Primary Care Physicians.

Demonstrates support for and participates in accomplishing department goals and objectives.

Must operate effectively with various levels of leadership and clinical expertise.

Functions independently with assigned duties, and as a member of a highly interactive team.

Assists team members with completion of duties in a timely and accurate manner.

Assists in various aspects of department process including but not limited to answering department incoming calls, securing and stocking department supplies, handling incoming and outgoing mail, scanning and faxing documents.

Maintains accurate and timely documentation and follow up when applicable.

Interacts with co-workers, visitors, physicians and other healthcare personnel in a manner that enhances service delivery and promotes positive relationships.

Behaves in accordance of the MHS Behavioral Standards.

Adheres to all HIPAA guidelines and patient confidentiality policies. Applies the minimum necessary standard when accessing protected health information.

Complies with established clinic policies and procedures and safety standards.

Performs other related work as required or requested.
