NaphCare is hiring an experienced Medical Discharge Planner just like you to join our team at the Wayne County Jail located in Detroit, MI. Pay rates range from $22.00-$30.00 per hour based on type of license, certification, Shift, FT, PRN, temporary contract, site needs, licensure, and experience.

Responsibilities:

Responsibilities for Discharge Planner:

Conduct rounds with physicians to meet with patients to discuss placement orders

Assist patients with a specific plan for medication management

Include patient in discharge planning

Assist patients with the identification of a support network, instructions for future care and follow-up

Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors

Qualifications:

Qualifications for Discharge Planner:

Bachelor’s Degree in a human service field

Minimum of 1-year experience in a clinical setting, with correctional healthcare experience preferred

Knowledge and experience in the development of discharge plans and coordination with community service providers

Valid CPR card
