How you make a difference:

The Social Services Care Coordinator is an integral part of the multidisciplinary treatment team and participates in the development and implementation of the treatment plan. This position is responsible for discharge planning and transition services. The Social Services Care Coordinator assesses individuals served from a psychosocial perspective, provides case management and ongoing assistance with benefits, community and facility resources, family work and education. The Social Services Care Coordinator provides therapeutic activities as appropriate.  This position is responsible for timely documentation of services that comply with legal, contractual, and regulatory requirements.

Key Responsibilities:

Conducts interviews with individuals, families, and stakeholders to gather background information and social history.

Participates in discharge planning and helps establish discharge plans.

Coordinates aftercare services, community linkages, and communicates with internal and external care systems.

Facilitates therapeutic activities as assigned.

Attends multidisciplinary team meetings and contributes to treatment plan development, while documenting service delivery and progress according to standards.

Qualifications & Requirements:

Education

Required: Bachelor's degree in social work, counseling, psychology or related field.

Preferred: Master’s degree.

Experience

Required: Minimum of relevant professional experience and/or training with relevant population.

Licenses/Certifications

Preferred: Licensure appropriate to discipline.
