Key Responsibilities:
Develop and implement individualized treatment plans that address the specific needs and goals of each client..
Deliver educational resources and ongoing support to patients and their caregivers, fostering better understanding and management of health conditions.
Act as a key liaison between patients, caregivers, and healthcare professionals, ensuring clear communication and coordinated care.
Understand each patient’s unique needs and preferences to provide a seamless and tailored experience, promoting adherence to treatment plans.
Keep thorough and up-to-date records, including progress notes, treatment plans, and assessments, to ensure compliance with clinical standards and regulations.
Offer confidential, non-medical counseling to patients experiencing chronic pain, helping them process their experiences and improve their well-being.
Requirements
Master'sdegree in Social Work from an accredited institution
Got a valid and active CA state license to practice as a Licensed Clinical Social Worker (LCSW)
Minimum of 2 years of clinical experience to the table, with a focus on providing psychotherapy services
Show off your knowledge of solution-based psychotherapy interventions and techniques.
Top-notch communication and interpersonal skills
