Overview:

M Health Fairview has an immediate opening for a RN in the Inpatient Care Coordination Department at the M Health Faiview Wyoming Lakes and Princeton Medical Centers.

This position provides a wide range of support and interventions across the Fairview system, to assist patients and families as they transition through an episode of care. This position supports patients and families in coping with the consequences of their illness and health care status. This position takes a leadership role in patient care, program and professional issues that they confront in their daily interactions, and supports the transition of patients along the health care continuum. Those within this department are working with social workers and fellow RNs to help with discharge planning needs. 

This is a casual/0.01 FTE opening meaning there are no guaranteed/scheduled hours per two week pay period and this position is not eligible for benefits coverage.

This position will cover the Fairview Lakes (Wyoming) and Northland (Princeton) locations.

Responsibilities/Job Description:

The incumbent will provide a wide range of services and interventions to assist individuals, their family, groups or communities to restore, improve, or protect their capacity for productive social functioning and coping with their illness and health care status. The individual is able to work independently and takes a role in patient care, program and professional issues that they confront in their daily practice with minimal supervisory mentorship.

Plans and implements treatment and transition care plans based upon a psychosocial assessment that is coordinated into an interdisciplinary care plan.

Provides a variety of interventions that include case management, counseling, crisis intervention, and advocacy in assisting patients and their support network reach their goals. Identifies and recognizes constantly evolving situations and quickly intervenes to prevent or resolve the emotional crisis.

Arranges, mobilizes, and may coordinate a comprehensive range of community resources which most optimally meet the identified patient needs.

Takes an active role in facilitation of interdisciplinary communication as it relates to patients experiencing sophisticated psychosocial issues.

Formally and informally educates patient/family  & health care team members regarding Patient Care Management Coordinator role, pertinent patient care issues, and population specific information.

Serves as a preceptor or field instructor for social work undergraduate training as appropriate to their skills related to a range of psychosocial issues and interventions.

Serves as patients advocate and liaison with physicians, support network and community to ensure continuity of care.

Documents psychosocial assessment, progress notes, care plans and discharge/transition plan in the electronic medical record.

Identifies and participates in Quality Improvement, research and program evaluation initiatives that focus on improving health care services

Demonstrates knowledge and compliance with MHealth Fairview and RN practice, ethics and standards.

Follows the legal and regulatory law including mandated reporting requirements.

Qualifications:

Required

AD in Nursing

5 years clinical experience

Active MN Registered Nurse license

Preferred:

BSN

1+ Years working as a care coordinator/case manager

Basic Life Support Certification (BLS)
