Job Description
The ideal candidate will work within a multidisciplinary team to provide physicals and medical care to existing clients received non-medical services and new clients as they join the practice. They will be expected to conduct initial Physical Assessment and plan ongoing care for children and adults.
They will be responsible to:
Conduct ongoing and wellness examinations and pharmacological evaluations
Prescribe medications, teach clients about side effects and monitor response to prescribed medications
Provide services to clients based on current status, risk, and as clinically indicated
Support staff to provide recovery-oriented and trauma-informed services
Participate in staff meetings
Provide crisis intervention services supporting staff as needed
Communicate with hospital ER attending when clients access emergency services
Support discharge planning with hospital and ER staff
Conduct risk assessment, provide guidance and direction on risk management
Use e-prescribing
Complete progress notes, significant events, referrals, and evaluations within expected timeframes
Provide individual counseling as part of the plan of care if and when needed
Use Electronic Medical Record (EMR) and data reports from EMR to track treatment progress and individual client outcomes
Provide crisis intervention services
Use collaborative documentation procedures for assessment and individual sessions with clients
Communicate with specialist and behavioral health team to deliver integrated treatment
Work in collaboration with the client and significant others to support recovery goals
