Job Description

The Mental Health Coordinator II (MHC II) is responsible to coordinate a variety of services including but not limited to individual finances, activities, program services, positive behavioral support needs, family involvement, education/vocational programs, health care/nutrition and household management.

What You Get to Do: 

Staff scheduling.

Assist with the supervisor in maintaining consistency in programming, individual care, and communication with staff and support team members.

Carry out the responsibilities and duties of the behavioral support staff during all direct care shifts.

Provide hands-on and/or verbal assistance, encouragement, mentoring, and guidance.

Ensure the comfort, safety, and personal growth of the individuals we serve.

Assist with daily living tasks such as moving around, getting in and out of bed, dressing, bathing, personal hygiene and more.

Light housekeeping tasks and meal preparation.

Build relationships with persons served and teammates.

Attend community occasions and fun activities like outdoor walks, games, and social events.

Work on tailored outcomes for individuals and work with behavioral guidance plans as needed per person served.

Transport persons served to appointments or other activities outside the home.

Empower people with disabilities to live life to the fullest.

Document progress, milestones, and action steps.
