Position Summary: *Remote Position*

The Case Management Specialist will be responsible for managing and coordinating the reporting, accountability, process improvement, and serve as an internal cross-functional liaison for specialized customer markets such as criminal justice and healthcare partners. The Case Management Specialist will support clinical teams at all BrightView sites by ensuring all external reports are completed and distributed on-time for patients from specialized customer markets; communicate with all functional teams such as outreach, operations, and clinical teams regarding the status of patients from specialized customer markets; prepare internal reports for BrightView that communicate overall status of patients by site, and metrics that track accountability for patients; work with external customer markets to better understand their needs, improve processes, customer service levels, and referral volume.

Responsibilities:

Essential Job Duties:

Represent "The BrightView Way" via our Core Values of Respect, Inspiration, Service, and Excellence or "RISE"

Interpret clinical notes and prepare accountability reports for criminal justice-involved patients

Prepare internal reports using CareLogic and excel to track specific metrics for criminal justice and other market

Ensure that all documents submitted on behalf of a patient are valid and authentic

Assure that documentation is completed in compliance with regulatory agency requirements and best clinical practices

Complete documentation utilizing the agency's designated Electronic Medical Record, within a specified timeframe

Serve as a subject matter expert on criminal justice and other markets for the purpose of business development

Develop and maintain a site by site reporting schedule to ensure reports are distributed to customers at the appropriate time

Collaborate with BrightView functional teams to ensure criminal justice and other specialized patients are properly accounted for

Survey customers and key BrightView staff to ensure we maintain service levels and continuously improve our processes

Maintain and continuously update a matrix of specific needs for each customer segment and subsegment (i.e., Segment- Criminal Justices, Subsegment- probation, parole, various courts and jail program types)

Attend required meetings including but not limited to, individual and group supervision

Complete required trainings including but not limited to, in-services, Relias trainings

Attend and plan meetings with partners to ensure proper engagement in the communities we serve and to build sustainable relationships with our partners

Attend and plan meetings with outreach, operations, and clinical teams regarding the status of our patients and our partners

Adhere to BrightView policies and procedures, and professional code of ethics

May provide various other services as needed

Qualifications:

Required Education/Degree:

Bachelor’s Degree preferred; Associate Degree may be considered                                                                                                                                                                            

Required Certification/License:

LCDC II or higher preferred; CDCA with the ability to advance to LCDC II may be considered

Other Requirements:

Must be highly self-motivated and able to work independently and as a part of a team.

Must have access to a phone and reliable high-speed internet in a location free from distraction to allow for adequate workflow and protection of PHI.
