We are offering a $1,000 Sign-On Bonus (to be paid throughout the first year of employment).

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

Rehabilitation services are dedicated to helping individuals achieve their highest level of independence and bridging the transition from illness to wellness at home and community, by emphasizing the restoration of mobility and function. The Physical Therapist provides quality Physical Therapy services including resident/patient/client assessment, evaluation, care planning, treatment and consultation in an interdisciplinary environment at Lutheran SeniorLife facilities/agencies including, as assigned, skilled nursing facilities, LIFE programs, home health and/or outpatient clinics.

This position will support Home Health, looking for someone with flexibility to cover multiple counties in Western, PA. 

Qualifications:

Bachelor's degree from an accredited four]year college or university with a degree in Physical Therapy; masters or doctoral preferred. 

One (1) to two (2) years’ related experience and/or training; or equivalent combination of education and experience. 

Experience working with senior population. 

Working knowledge of computer software required for documentation and communication. 

Maintains current PA license as Physical Therapist including meeting continuing education requirements.
