Qualified Social Worker required to join a statutory Hospital Discharge in line with Discharge to Assess Team
Office base: University College Hospital London
To work Friday, Saturday and Sunday and Bank Holidays.
Friday - based in the hospital.
Sat and Sun - mainly remote working.
Candidate should have experience of Hospital Discharge in line with Discharge to Assess.
Knowledge and skills in line with Care Act responsibilities. To be considered, you must have a social work qualification and full SWE registration as well as over 3 years post qualified experience gained in an Adults Services setting
