It is amazing when a career and a passion come together! AdvisaCare Home Health Care is seeking a Care Manager in our East Lansing Office. A Licensed Practical Nurse/LPN license is required for this position. This is a full time, salary position with a competitive benefit package including medical, dental, vision, bonus plan and a 401K!

As the Care Manager , you will utilize your nursing and management skills combined with your compassionate talents. This position affords an excellent opportunity to have an impact on seniors and their families. Our ideal candidate will appreciate challenges, have the ability to continually adjust to change and multitask, have pre-existing knowledge of nursing and management, all with a great attitude.

Care Manager relationships are key, whether it is providing care to the residents, encouraging residents to strive each day to live independently without assistance, supporting families, or assisting associates. Each day will have different challenges and emotional rewards.

Requirements

The primary purpose of the Care Manager is to plan, organize, develop and direct the overall functions of the nursing services and to ensure the highest degree of quality care is maintained at all times.

Other duties include:

Direct the nursing services department including hiring, training, scheduling and coaching staff

Manage the daily nursing functions

Communicate with health care providers, residents and family members

Qualifications:

Must possess a current and active license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)

Must have, as a minimum, 2 years of experience in nursing. Preferably experience in a facility environment and/or skilled home care

Experience in supervision and principles and practice of teaching

Strong leadership and communication skills

Possess creativity, integrity, and initiative

Ability to provide critical thinking to address clinical issues
