Connect2socialwork are urgently looking for a Social Worker to join their great Adults Mental Health Team with a Pay rate up to £38 an hour on a locum basis.

Assessment and Care Planning
Conduct thorough assessments of individuals' mental health needs, including risk assessments and evaluations of social, emotional, and environmental factors.
Develop and implement individualized care plans in collaboration with service users, their families, and other professionals, focusing on recovery-oriented goals.
Direct Support and Intervention
Provide therapeutic support, counselling, and crisis intervention to individuals experiencing mental health issues.
Advocate for service users' rights and needs, ensuring they have access to appropriate services and resources.
Multi-Agency Collaboration
Work closely with healthcare providers, community services, housing agencies, and other relevant organisations to coordinate and deliver integrated care.
Participate in multidisciplinary team meetings, case conferences, and care reviews to ensure comprehensive and cohesive support.
Safeguarding and Risk Management
Identify and respond to safeguarding concerns, following established protocols to protect vulnerable adults from harm.Requirements as a Social Worker include:

SWE Registration
Eligible to work in the UK
Drive and have access to a car
Post-qualified experience
