As a Home Health Physical Therapist Assistant at AdvisaCare, you will work closely with patients in their homes, providing assistance with their treatment plans under the direction of a licensed Physical Therapist. You will be an essential part of our team, helping to support the care of individuals and families within the community in which you live!
Responsibilities
Implement physical therapy treatment plans as directed by a licensed Physical Therapist
Prepare and maintain accurate and timely clinical documentation
Assist patients with exercises and activities to promote strength, safety, and mobility
Provide education to patients and their families about their conditions and treatment plans
Ensure a safe and effective home environment for clients
Collaborate with interdisciplinary team members, including nurses, physical therapists and occupational therapists, to coordinate patient care
Requirements
Current state licensure in Michigan as a Physical Therapist Assistant
Minimum of one year of experience in home health care is preferred, but not required
Strong communication and interpersonal skills
Ability to work independently and manage multiple patients effectively
Valid driver's license and reliable transportation
