Minimum Qualifications:

Must be licensed as a  Physical Therapist in the state of Oregon

Graduate from an accredited program with a degree in Physical Therapy

CPR Certification

National Provider Identifier (NPI) required upon hire

Preferred Qualifications:

Experience in Home Health

Organizational skills to facilitate program development

Strives to positively represent their discipline and AgeRight Home Care in the community

Responsibilities:

Complete assessments for every patient upon admission

Establish treatment plans

Teaches patient and family/caregivers in meeting individual treatment goals, and treatment plan implementation
