As part of a multidisciplinary team to deliver patient-centered nursing and rehabilitation services to patients. Support the Unit Manager to maintain excellent clinical standards and patient experience. Provides the highest level of service to all patient's, families, and visitors to ensure satisfaction. Responsible for ensuring all assigned residents receive all medications, treatments, and nursing care as ordered. Completes all required documentation as per state/federal guidelines.

Responsibilities:

A day in the life of a Licensed Practical Nurse at Hackensack Meridian includes:

Administering medications and treatments as per physician orders

Assisting in the supervision of nursing assistants

Assisting with developing and implementing plans of care for residents

Ensuring that documentation meets all state/federal guidelines. Documentation indicates accuracy of charting:

Plan of Care

Interventions/TX

Administration of meds

Teaching

Ensuring all medications/treatments of assigned residents are given as ordered

Performing and assist with direct patient care as needed

Ensuring resident has proper equipment and adaptive devices as ordered

Co-ordinating resident care with other facility departments

Assessing and identify any changes in resident condition and needs and report to charge nurse

Qualifications:

Completion of an approved LPN program

NJ LPN License

Current with all Continuing Education (CE) requirements 

Serve as patient advocate and role model to other Team Members

Deliver patient care with minimal disruptions and obstacles

Ability to effectively work on a multidisciplinary team

Long-term care facility experience preferred 

Prior work experience with geriatric and disabled populations preferred 

Bi-lingual (Spanish) preferred

CPR Cardiopulmonary Resuscitation preferred

IV Certification preferred

BLS Basic Life Support preferred
