The Specialty Clinical Pharmacist assists the Clinical Coordinator and Pharmacy Manager in developing relationships with key HMH clinics to identify and meet customer needs. Head patient medication therapy management teams in a broad range of disease states, providing comprehensive and individualized care to the patients in their assigned areas. Monitor and improve medication adherence via therapy management, offer more extensive and recurring educational and counseling sessions to improve patients understanding and adherence of their therapy regimen, and may provide resources for additional financial and emotional support. Counsel patients on critical factors that include dosage, duration, conditions, adverse events, drug interactions, and other monitoring parameters, of complex drug therapy regimens, to maximize clinical outcomes. Effectively communicate with prescribers and patients to ensure optimum care. Support Clinical Coordinator and Pharmacy Manager with report findings and develop implementation strategies together with operations staff to meet clinic needs and provide growth opportunities for HMH Specialty Pharmacy.

Responsibilities:

A day in the life of a Specialty Clinical Pharmacist at Hackensack Meridian Health includes:

1. Lead a patient medication therapy management team including clinic liaison technicians and pharmacy technician navigators

a. Monitor and improve medication adherence via therapy management system

b. Offer extensive and recurring educational sessions of complex drug therapy regimens to maximize clinical outcomes

2. Work together with Clinical Coordinator and Pharmacy Manager and staff, as appropriate, to implement new programs and services in response to clinics needs

a. Participate in the instruction and supervision of technician externs and pharmacist interns

b. Participates in department projects or committees where needed

c. Act as a key drug information resource to physicians, nurses, and patients

3. Develop relationships with key HMH Health System clinics. 

a. Identify decision makers, establish working relationships and build rapport with them

b. Develop and maintain policies and procedures for specialty pharmacy services in order to assure high quality pharmaceutical care.

c. Develop a plan/timetable to monitor clinics on a regular basis. This includes reporting on quality and performance indicators, such as revenue, number of prescriptions, margin, capture rate, phone metrics, consumer satisfaction, prior authorization approval rate, time, and turnaround time.

4. Identify new specialty pharmacy opportunities based on current restricted payer networks, current limited distribution drugs, pipeline drugs and all other specialty pharmacy opportunities.

a. Bring information gathered to pharmacy manager, as appropriate, for review, evaluation and prioritization of the opportunity.

5. Participate in developing regional strategic planning and integration efforts

a. Tracks trends and outcomes measures.

b. Responsible for local implementation of system wide quality assurance, error reduction and customer satisfaction programs. Identifies opportunities and leads efforts for improvement to achieve best-demonstrated practice.

c. Has a clear vision of the future of the business unit and works actively with all internal and external customers to fulfill this vision.  Effectively implements strategic plan and milestones.  Effectively evaluates role of technology and recommends plans to technologically advance department.  Identifies resources needs (people, equipment, facilities) to support implementation of plans.

6. Work together with Clinical Coordinator and Pharmacy Manager to develop analytic reports and dashboards required for URAC, ACHC, and any other specialty pharmacy accreditation.

7. Monitor the market regarding changes in therapies and reimbursement that will impact pharmacy revenue referred from clinics.  Stay abreast of new technology and developments in innovation that will impact HMH clinics; and identify those which would create favorable revenue/margin opportunities for HMH Specialty Pharmacy.

8. Complies with departmental policies regarding safety, attendance, and dress code.

9. Other duties as assigned

Qualifications:

Education, Knowledge, Skills and Abilities Required:

1. Pharm.D.

2. At least three (3) years of previously effective pharmacist experience in Hospital, Retail, or Specialty Pharmacy.

3. Must be comfortable using multiple software programs.

4. Must be organized and possess effective time management skills.

Education, Knowledge, Skills and Abilities Preferred:

1. Specialty Pharmacy

2. Epic/Willow Ambulatory

Licenses and Certifications Required:

1. Registered Pharmacist License.

Licenses and Certifications Preferred:

1. Board Certified and/or Certified Specialty Pharmacist
