Responsibilities

Conduct initial assessments of clients' needs and develop personalized care plans

Coordinate and schedule home care services based on clients' requirements

Collaborate with clients, their families, and healthcare professionals to ensure the delivery of high-quality care

Monitor the implementation of care plans and provide ongoing support and guidance to caregivers

Assess caregiver performance and provide feedback and training as needed

Maintain accurate and up-to-date documentation of all client interactions and caregiver assignments

Stay informed about industry trends and best practices in home care services

Requirements

Bachelor's degree in a related field or equivalent work experience

Minimum of 2 years of experience in home care coordination or a similar role

Proven ability to assess clients' needs and develop customized care plans

Excellent organizational and time management skills

Strong interpersonal and communication skills

Ability to work independently and as part of a team

Proficient in using computer applications and software for documentation and scheduling
