Responsibilities:
The primary purpose of the position is to assist in the identification, monitoring, engaging, and linking of patients to appropriate services. The patient population identified will be those at risk for adverse health outcomes, those with limited healthcare resources and those with difficulty accessing services. Supports the mission of University Hospital.
Qualifications:
High School Diploma and one (1) year of experience in a medical setting are required, ideally with pregnant patients or those with substance misuse disorders in those with a uterus; two (2) years is preferred. Familiarity and experience with Microsoft Office Suite preferred. Bilingual a plus. May be required to follow a varied schedule which may include evenings, weekends, and holidays. Required possession of a valid NJ driver’s license. Must possess excellent written and verbal communication skills.
